Here are commonly requested documents, forms, and links.  If you don’t see the information you are looking for here, please visit the Cusick website.

Residents that own a business can create a business card for the Business Directory.   Using the Brookhaven Business Directory is a great way to support your neighbors and small businesses!

Brookhaven is on the “Green” yard waste schedule and the “Orange” recycling schedule.

The Clubhouse may be used as a remote workspace during normal business hours.  A reservation is required. Paid or community events take precedence. You may not use any part of the Clubhouse if it has been reserved by a resident or for a community event. See the Coworking Policy page for details and the required waiver.

The Main Floor, Lower Level, and Entire Clubhouse are available for rental by residents for events and parties. For details, please visit the Clubhouse Rental page.

Brookhaven HOA management is provided by Cusick Community Management.

  • For instructions on setting up or updating your existing Brookhaven website account, click here.
  • If you are a new resident you will need to create a new account by completing the New Resident form.
  • If you have previously logged into your account but have forgotten your username or password, use the “Retrieve username & password” link on the login screen.
  • Complete the Amenity Card Service Request form if:
    • You have a key fob from a previous homeowner that needs to be activated for you.
    • Your key fob does not access the fitness center.
    • You need pool access for early morning lap swimming.
    • You lost (or never had) a key fob and need a new or replacement one.
  • For any other key fob issues contact Cusick Community Management by e-mail at communitysupport@cusickcompany.com

Complete the Amenity Card Service Request form below in order to authorize your key fob for pool entry during lap swim hours.

HOA meeting minutes are published on the Cusick website under Community Information\Community Documents\Documents\Open Minutes.  Minutes are approved during and published after the next scheduled meeting following the meeting where the minutes are recorded.

Residents who opt in to have their contact information published are listed in the Neighbor Directory.  The directory is located under the Resident Resources menu and on the Manage Account screen. The directory is only visible to residents who are logged in, the information is not made public. You can update your preferences by going to Manage Account and using the Member Directory toggle to turn on/off publication of your contact information.

  • To pay your dues, review your balance, or set up recurring payments, please visit the Cusick Community Portal at https://cusick.cincwebaxis.com.
  • Assessments will show on the pay assessment screen the date they are due.  If you do not see this, find the tab labeled Account information, and click on it. Once there you will see a homeowner ledger pop up. Located at the top there is an option to change the dates so you can see upcoming and previous assessments and payment amounts.
  • For assistance with payments or to set up ACH payments, please contact the Cusick accounting department at accounting@cusickcompany.com.

Residents are allowed to host pool parties during normal pool hours.  Please see the Pool Party Rules and Policies for costs and instructions.

Complete the Issue Notification form below to report an issue to Cusick, such as a common area maintenance need or a covenant violation.

  • All changes, modifications and/or additions to the exterior of your home OR property MUST BE approved by the Architectural Review Committee (ARC) via an ACC Request Form. The ACC request form can be found on the Cusick website.  After you sign in to the Cusick website you will be on a Dashboard page and the bottom right tile will say “Architectural Requests”.  Click the “See All” button and then “Add ACC Request” to submit a new request.
  • To avoid unnecessary delays in the approval process, be sure to include as much detail as possible such as PROPERTY SURVEYS, PICTURES, ARCHITECTURAL RENDERINGS, MATERIALS USED, COLORS, etc.
  • A survey is not needed for a paint color change since it does not impact any setbacks, property lines, etc.
  • If you have any questions about the architectural changes that are allowed, please review the Architectural Guidelines (including allowed paint and trim colors) on the Community Documents page located on the Cusick website.  Select Community Information\Community Documents from the Cusick menu, and then expand the Community Documents folder.   • For tennis court reservations, click on Reservations\My Reservations on the Manage Account screen to view, edit or cancel an existing reservation.  The link for court reservations can also be found under the Resident Resources menu and on the Tennis Amenity or Activity page. There is no fee to reserve a tennis court.
  • If you have paid the annual Equipment Use Fee for use of the tennis ball machine and other equipment, the ball machine is also reserved on the Make Reservations tab of the Manage Account screen, or any page where court reservations are made.  Don’t forget to also reserve a court for the same time.  For more information on use of the tennis equipment please visit the Tennis Courts page.

Each year a banner is displayed in front of the Clubhouse congratulating our graduating Brookhaven seniors.  Sign-ups for the banner typically begin in January and end in March.  When open for sign-ups, the form can be found here.

Use the Union County Street Light Repair form to notify the county.  You will need the pole number located on the street light in order to complete the form.
