The Amenity Card Request form is for Brookhaven residents only. You must be logged in to complete the form.
Use the Amenity Card Request Form to request new key fobs or to add access, including Lap Swim, to an existing key fob.
You will not have access to the form if you have a new account in a Pending Review status, or if you are not the homeowner. Renters are granted the same access to neighborhood amenities as homeowners, but the homeowner is required to complete the Amenity Card Service Request on behalf of their renter.
