Action Requested – Agreement for Traffic Light Installation

Dear Brookhaven Homeowners,
We are reaching out to request your response regarding the dedication of a portion of the common area for the installation of a traffic light. The attached document provides detailed information about this initiative.
How to Submit Your Agreement:
Please review the attached document (link also below), complete the written agreement, and return it using one of the following methods:
- Email: Take a picture of the completed agreement and email it to Meredith Johnson Emerson at memerson@cusickcompany.com.
- Mail: Send the completed agreement to:

Important Note: If more than one owner is listed on the deed, all owners must sign the agreement for it to be valid.
Deadline for Submission: April 28, 2025
A total of 375 homeowners must approve the agreement for the traffic light installation to move forward.

If you mail your document, you will not receive a confirmation of receipt. If you submit your document electronically via the link or QR code, you will receive a confirmation from JotForm if your form was submitted successfully. You will not receive a confirmation from Cusick. If you emailed a picture of your signed document directly to Meredith, you will receive confirmation of receipt but it will not be immediate.
